Lower Extremity Amputation Prevention Project

This project will compare two alternative approaches to helping patients at risk for amputation (those with underlying neuropathy or peripheral arterial disease who have a "foot attack", i.e. a limb-threatening problem) to receive the care they need, when and where they need it. This may be directly from their primary care provider, or at a referral center. There would also potentially be a telecommunications outreach element to this to build the referral network, possibly using the ECHO approach. Outcomes would be monitored using EPIC, and REDCap would be used to collect any additional patient-reported information that patients/providers decide is important to include.
